OFFICE VISIT INSTRUCTIONS
To make the most of the limited time available for in office visits
-Please bring your adult MSP card and picture ID and the MSP number for the baby.
- Please bring 2 clean disposable diapers and a light blanket to cover the baby.
- If you are using any bottles, SNS, nipple shields other feeding devices please bring those too.
- Please bring babies birth weight and recent weights, and the name and dosages of any medications either of you are taking
